I. Aeromedical Examination at OFSAM
Aeromedical examination at OFSAM is on a controlled capacity and request for appointment schedule shall be made online on a first come, first served basis.
Only airmen applicants with confirmed appointment shall be allowed entry to OFSAM facility. To ensure the safety of authority personnel and other clients/guests, mandatory temperature screening, wearing of facemask and physical distancing are strictly implemented. Filling out and submission of Health Declaration Form on the day of scheduled appointment is also mandatory.
Should an airman developed any of the COVD-19 symptom/s within 14 days before the confirmed scheduled appointment, concerned airman must inform the Aeromedical Section thru email or call tel. nos. 7944-2122 to 23 for cancellation of appointment and make another appointment once fully recovered.
II. Deferment of Medical Examination and Medical Examination by a Licensed Medical Practitioner Other Than CAAP Designated Aviation Medical Examiner (DAME)
Due to continuing limitations brought about by the public health crisis, deferment of medical examination and applicable medical examination by other licensed medical practitioners are temporarily allowed until further notice pursuant to ICAO Annex 1 Part 1.2.5.2.6.

The front page of the application form shall be completely filled out and signed by the airman applicant while the back page by the examining physician. The Eye Examination Report shall be completely filled out and signed by the examining eye specialist or optometrist.
Note: Incomplete entry/entries in the form/s may result to delay of medical report evaluation and may be a ground for disqualification.
III. Schedule of Routine Tests Requirement

A. CLASS 1 MEDICAL CERTIFICATE

After physical/medical examination of the examining licensed medical practitioner, sealed application package consisting of completed forms and test requirements shall be submitted by the concerned airmen applicant within 10 working days at OFSAM Receiving Window or may mail package to this address:

IV. COVID-19 REPORTING AND MEDICAL ASSESSMENT
Following the recommendations of public health authority and other pertinent authorities, SUSPECT, PROBABLE OR CONFIRMED case of COVID-19 must stop flying or controlling until cleared by health authority.
All holders of aviation medical certificates who tested positive for COVID-19 on Swab Test (RT-PCR) must report to Aeromedical Section thru phone or email for purposes of monitoring and recording as necessary for medical assessment by Aviation Medical Examiners (AMEs). Information will be strictly confidential under the PCAR Part 2.10 and Data Privacy Act, subject to known legal exceptions.
If the diagnosis is uncertain or not available (e.g. symptoms managed at home without testing), holders must cease flying/controlling, and seek medical advice from medical practitioner as appropriate.
V. VALIDATION OF FOREIGN MEDICAL CERTIFICATES
Airmen applicants requesting Validation of Foreign Medical Certificate must submit the following requirements at OFSAM Receiving Window:
- Scanned copy or printed photo of valid Foreign Pilot License;
- Scanned copy or printed photo of valid Foreign Medical Certificate verified and endorsed by designated LCD – FSIS pilot examiner;
- PEL Number of airman applicant is required to facilitate entry to CASORT and avoid delay; and
- Official receipt as proof of payment.
VI. RE-ISSUANCE OF AVIATION MEDICAL CERTIFICATE
Airmen applicants requesting Re-Issuance of Aviation Medical Certificate must submit the following requirements to aeromedical@caap.gov.ph or at OFSAM Receiving Window:
- Scanned copy or printed photo of valid Pilot License;
- Letter Request addressed to Chief, OFSAM;
- Affidavit of Loss for lost Aviation Medical Card; and
- Official receipt as proof of payment.

VII. EXTENSION OF MEDICAL CERTIFICATE
Approval of request for extension of medical certificate is at the discretion of the Authority pursuant to PCAR Part 2.10.1.8.c(v) and applies only to those holders of valid medical certificate. No appointment schedule is required. Letter request for extension must be addressed to the Chief of OFSAM and sent to aeromedical@caap.gov.ph prior to the date of medical certificate expiration.
VIII. CANCELLATION AND RE-SCHEDULING OF APPOINTMENT
Airmen applicant who cancelled his/her medical/physical examination or failed to appear on scheduled appointment at OFSAM must make another request for appointment schedule.
IX. PAYMENT OF FEES AND CHARGES
Payment of fees and charges will be at CAAP Central Office and Area Centers while awaiting completion of the online payment scheme process.
X. EFFECTIVITY
This Advisory shall take effect immediately.
